Community Garden Affiliate Renewal Form

This form serves as a way to update your garden’s contact information, collect details about this season, and inform us of changes and needs you may have in the coming year. Please complete this form in its entirety.

This year renewal forms are available to gardens both electronically and in paper form. Gardens only need to complete one renewal form (electronic OR paper, not both).

Due to the high number of Five Rivers MetroParks Community Gardens, all gardens must submit a renewal form by the deadline in order to secure their spot with the Community Gardening Program for 2018. Gardens that do not submit a renewal form by the deadline will jeopardize their standing in the program, which may include lack of access to services and resources.
